Care Harmony is seeking an experienced Licensed Practical Nurse (LPN/LVN) to join our remote intake coordination team. You will work with providers to deliver value-based care management for patients, conduct initial health assessments, identify resources, and coordinate needs for chronic care management.
The role offers a fully remote schedule, Monday–Friday, with no weekends and minimal on-call rotation. A strong license, communication skills, and a passion for patient care are required.
