Virta Health is seeking a nurse practitioner leader to mentor a team of 10-12 Continuity Clinicians while seeing a part-time patient panel (0.2 FTE). This role emphasizes coaching, performance improvement, and delivering exceptional care and outcomes for Virta members.
The CC lead, reporting to the Director, Provider Operations, will partner with VMG leaders to identify opportunities to improve team performance and support adoption of organizational priorities and initiatives.
