Location: Wilkesboro
Gentiva Hospice in Wilkesboro, NC seeks an RN Case Manager to lead home-based patient care, coordinating nursing interventions and documenting plans of care. You will perform assessments, collaborate with the IDG, and support patients and families with education and bereavement guidance.
Requirements include RN license in NC and CPR certification; hospice experience is preferred. We offer a $5,000 sign-on bonus and comprehensive benefits.
