Nurse Practitioner - Hybrid Role

Our comprehensive program incorporates Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) to prioritize proactive measures for enhanced physical health and well‑being.

We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high‑quality preventative care to residents in skilled nursing facilities. This hybrid role offers flexibility to work on‑site and remotely, supported by a dedicated scribe team for efficient documentation.

• Build strong relationships with facility staff and patients while maintaining a patient‑centered approach
• Collaborate with a multidisciplinary team to provide comprehensive care
• Conduct on‑site visits and remote work from home
• Document patient care efficiently with the support of a dedicated scribe team
• Participate in education and training programs to stay up‑to‑date with industry developments

• Active NP or PA license in the state of practice
• 1+ year of experience in geriatric care and documentation review
• Excellent communication, collaboration, and teamwork skills
