Elk Valley, part of LHC Group and a member of the Optum family, seeks a Home Based LPN to provide direct nursing care in patients’ homes in the Shelbyville region. You will document progress notes and coordinate ancillary services under a supervising RN.
Responsibilities include high-quality clinical care, charting in the EMR, care coordination, and effective communication with patients, families, and the care team. A valid LPN license and reliable transportation are required, with CPR preferred.
