DaVita IKC RN Case Manager (National Flex Team)
This position supports DaVita Integrated Kidney Care (DaVita IKC), providing remote case management for ESRD and CKD patients across the U.S. The RN Case Manager works autonomously and collaborates with the healthcare team to coordinate quality, cost‑effective care and minimize fragmentation of the delivery system.
Key Responsibilities- Coordinate care between patients, families, physicians, providers, care teams, community resources, and Regional Operations Manager.
- Facilitate quality, cost‑effective care while minimizing fragmentation in the delivery system.
- Collaborate with multidisciplinary teams to ensure seamless care transitions and optimal clinical outcomes.
- Bachelor’s or Associate’s degree in nursing from an accredited program (B.S.N. preferred, A.D.N. acceptable).
- Minimum one year of DaVita IKC RN Case Manager experience required.
- Current compact nursing license in the state of employment.
- Minimum of five years clinical nursing experience, with at least three years renal nursing experience.
- Proficiency in MS Word, Outlook, and DaVita clinical software (Capella) within 90 days of employment.
- Current CPR certification.
- Current driver’s license and positive driving record.
- Strong communication and computer skills.
- Ability to adapt practice patterns to new electronic processes and meet productivity expectations.
- Certified Nephrology Nurse (CNN) or Certified Case Manager (CCM).
- Post‑secondary education in quality improvement (CQI) or experience as a change agent.
- Proficiency in post‑secondary training; PST or MST preferred.
