We are seeking a dedicated and compassionate Registered Nurse (RN) to join our growing home care team. This full-time/part time RN position focuses on completing client assessments and reassessments to ensure high-quality, individualized care for patients in the Northern Virginia communities. The ideal RN is someone who has a passion for nursing, and great customer service skills.
Responsibilities- Conduct initial client assessments and routine reassessments in client home settings
- Conduct hospital discharge assessments
- Develop and update individualized care plans
- Ensure compliance with care standards
- Maintain accurate and timely documentation
- Currently licensed Registered Nurse (RN) in state of Virginia
- Previous clinical experience (home care, case management, or patient assessment) as a Registered Nurse is preferred
- Ability to work independently
- Excellent interpersonal and client care skills
- Valid driver’s license
