Gentiva Hospice is seeking a Nurse Practitioner to deliver advanced, patient-centered hospice care in patients’ homes and community settings. You will conduct comprehensive assessments, develop individualized care plans, and collaborate with physicians, nurses, social workers, and chaplains to align care with patient and family goals.
You will perform face-to-face evaluations, obtain histories, order tests as needed, and prescribe treatments within regulatory guidelines.
