Care Talk Health is seeking experienced Registered Nurses for a fully remote, contract (1099) role to support virtual care programs. You will perform AWV chart preparation, conduct telehealth visits, and build comprehensive care plans across CCM, RPM, RTM, and APCM, coordinating with providers and patients to close care gaps.
Ideal candidates have 2–3 years of clinical nursing experience, an active RN license in a compact state, and proficiency with EHRs and telehealth platforms.
