RN Case Manager Experienced; IKC

Job Description

This DaVita IKC Nurse position serves the Harrisburg, York, and Western Philadelphia region and is a hybrid remote/field-based role.

The position functions autonomously and collaborates with the healthcare team to coordinate quality, cost-effective care for ESRD and CKD patients while minimizing fragmentation of the delivery system.

Coordination of care includes working with patients, families, physicians, providers, care teams, community resources, and Regional Operations Manager.

Positions can be telephonic, field-based, or a combination. Field-based roles require a home office and travel to dialysis centers within the designated area. Telephonic roles are remote and require a home office.

• A.D.N. degree from an accredited school of nursing required; B.S.N. preferred; a three-year diploma from an accredited diploma program may be substituted.
• Current Registered Nurse license in the state of practice required.
• Continuing education credits maintained as required by state of practice.
• Minimum of five years’ experience in clinical nursing.
• Minimum of three years’ experience in renal nursing preferred.
• Demonstrated knowledge and understanding of data and managing it to improve clinical, financial, and patient satisfaction outcomes.
• Demonstrated experience and effectiveness in a change-agent role.
• Demonstrated knowledge and understanding of CQI techniques.
• Previous experience in healthcare performance coaching required.
• Certified Nephrology Nurse (CNN) or Certified Case Manager (CCM) preferred.
• Current CPR certification required.
• Ability to modify personal practice patterns to adapt to new electronic processes and increased productivity expectations.
• Current driver’s license in state employed with a positive driving record and required insurance coverage.
• Basic computer skills and proficiency in MS Word and Outlook.
• Functional proficiency with DaVita specific clinical software programs, including Capella, required within 90 days of employment.
• Home office with internet connectivity at minimum of 1 MB upload and 1 MB download speed.
