RN Case Manager, Savannah, GA- Hybrid Remote

DaVita Integrated Kidney Care is the leading value-based care provider for patients with kidney disease.

• Location: Savannah, GA
• Schedule: Monday–Friday, full-time schedule with the ability to accommodate patient availability. No nights, No weekends, No on call.

• Assist complex patient populations in navigating the healthcare system and advocating for them.
• Support patients holistically, focusing on patient engagement, connection, and case management.
• Coordinate care between the patient, family, physician, provider, care team, community resources, and Regional Operations Manager.
• Provide quality, cost-effective care while minimizing fragmentation of the healthcare delivery system for Chronic Kidney Disease (CKD) and End-Stage Kidney Disease (ESKD) patients.
• Educate patients, foster confidence in navigating their care, and promote positive outcomes.
• Adapt to new or changing technology to improve care coordination.

• Degree from accredited school of nursing required; B.S.N. preferred.
• Current Registered Nurse (RN) license in the state of practice required.
• Continuing education credits maintained as required by state of practice.
• Minimum of five (5) years’ RN experience in clinical nursing.
• Previous case management experience required.
• Demonstrated knowledge and understanding of data and its use in clinical, financial, and patient satisfaction outcomes.
• Experience and passion for educating patients.
• Current CPR certification required.
• Ability to adapt to new or changing technology.
• Current driver’s license in state employed with a positive driving record and able to meet requirements of insurance coverage.
• Basic computer skills and proficiency in MS Word and Outlook required.

Salary range: $68,400.00 – $ per year.
