• Provide admission, evaluation, and development of individual care plans for clients of all ages in the home setting.
• Administer medications and nursing treatments per physician orders; monitor patient status and respond to changes.
• Collaborate with a multidisciplinary team of home care professionals and paraprofessionals to ensure coordinated, patient-centered care.
• Educate patients and families on disease management, medication use, wound care, safety, and self-management strategies.
• Maintain thorough documentation of assessments, interventions, and outcomes; ensure accuracy and compliance with standards and regulations.
• Promote infection control, safety, and quality-of-care practices across all visits and transitions of care.

• Active Vermont Registered Nurse license
• Minimum 1 year of prior clinical experience
• Ability to work in a home health setting and travel to patient locations
• Strong communication and teamwork skills
• Ability to work the Days shift and manage a traveling RN schedule in a home-based environment
