Apex Family Healthcare Services is an Outpatient Mental and Behavioral Healthcare Agency offering services and programs for Adults, Children and Adolescents located in Stockbridge, GA. We extend our services to individuals throughout the Greater Metro Atlanta area through our mobile program. JobObjective
RN is responsible for providing oversight and/or coordinating medical on-site or in-home services to consumers residing within a 50-mile radius of Stockbridge, GA. Applicant will provide recovery services focused on community-based care for individuals with mental health diagnosis and co-occurring issues. Perform medical care and health screenings. Responsible for the ongoing assessment of health status to ensure timely and effective interventions in the preventative and/or maintenance healthcare.
Qualifications- Current Georgia RN License
- At least 1 year experience in working with the BHDDAD population
- CPR/AED/First Aid certification
- Current driver's license, valid vehicle insurance and access to a dependable vehicle, or public transportation.
- Conduct in-office and in-home visits and assessments.
- Supervises self-administration of medication
- Manages lab results, drugs, etc. and share information with physicians
- Assures proper method; time and dosage are indicated by established individualized medication record and/or nursing plan
- Explains medication and treatment to consumer and/or guardian
- Observes and records consumer’s response and/or medication side effects
- Interviews consumers or family members to obtain a thorough medical history.
- Takes vital signs and performs other tests as required by following established guidelines.
- Implements physician’s orders and updates medical care as necessary.
- Participates in team conferences, sharing knowledge, information and ideas with team members.
- Identifies, plans, coordinate community-based services for members.
- Attends and participates positively in all staff training activities and seek opportunities to improve own competencies and professional knowledge, as necessary.
Negotiable range based on experience.
