Humana Inc. is seeking a Telephonic Nurse 2 Care Manager to support members with chronic conditions from a home office. You will assess needs, develop care plans, and coordinate with a multidisciplinary team to improve health outcomes.
This remote role requires bilingual Spanish/English communication, strong nursing judgment, and the ability to manage multiple systems. Training occurs first four weeks, then 10:00 AM–6:30 PM EST with limited day-to-day schedule flexibility.
