Care Talk Health is seeking an experienced Registered Nurse for a fully remote contract role. The RN Care Manager will handle AWV chart preparation, care management across CCM/RPM/RTM/APCM, and care gap closure while supporting providers in a telehealth environment.
Ideal candidates have 2–3 years of nursing experience, strong documentation and communication skills, and the ability to work independently from a private home workspace. This is a 1099 remote contract across multiple states.
