First Health Hospice & Palliative Care is seeking an RN Case Manager to deliver skilled hospice care in patients' homes. You will work with the interdisciplinary team to implement and oversee care plans, focusing on symptom management and family education.
Two years of acute care or home care experience is preferred, along with a valid NC driver’s license. CHPN certification is encouraged after two years of service.
