Inova Health System is seeking an experienced RN Case Manager I to join ICPH. This remote role begins with onsite training at ICPH, then becomes fully remote.
The RN will perform discharge planning, coordinate services, and liaise with patients, families, and care teams in both acute and post-acute settings. The role requires a Virginia RN license (or eligibility) and at least 1 year of case management or clinical care experience; BSN plan may be required for progression.
