Registered Nurse Hospice Case Manager
Registered Nurse Hospice Case Manager provides skilled nursing care and coordinates interdisciplinary teams for patients receiving home-based hospice care in the communities south of Salt Lake City. The role manages a caseload of patients, develops individualized care plans, and ensures compliance with Medicare hospice regulations while supporting patients and families through end-of-life care.
Key Responsibilities:- Perform comprehensive hospice admissions and ongoing patient assessments
- Develop, implement, and revise individualized hospice plans of care
- Coordinate care with physicians, hospice aides, social workers, chaplains, therapists, and other interdisciplinary team members
- Provide skilled nursing care focused on pain management, symptom control, and patient comfort
- Educate patients and family members regarding disease progression, medications, hospice services, and end-of-life care
- Monitor patient status and adjust care plans based on changing clinical needs
- Coordinate necessary medical equipment, medications, and community resources
- Complete accurate and timely electronic documentation
- Participate in interdisciplinary team meetings and case conferences
- Ensure compliance with Medicare hospice regulations and agency policies
- Provide emotional support and advocacy for patients and their families throughout the hospice experience
- Current RN license in Utah or eligible for licensure
- Graduate of an accredited nursing program
- Minimum one year RN clinical experience
- Strong clinical assessment and critical thinking skills
- Excellent communication, empathy, and interpersonal abilities
- Ability to work independently while managing a patient caseload
- Valid driver's license with reliable transportation and current auto insurance meeting state requirements
Skills:
- Previous hospice, home health, palliative care, oncology, critical care, or case management experience
- Experience with electronic medical records (EMR)
- Hospice and Palliative Credentialing Center (CHPN) certification
