Nurse Practitioner; Home Based Primary Care

Overview

The Nurse Practitioner (Home Based Primary Care) works in Geriatrics and Extended Care Service to provide a full range of services emphasizing quality, timeliness, and the VA mission. The NP provides advanced clinical practice, consultation, and management.

• Independently manage a panel of patients with acute and chronic health conditions, emphasizing health promotion and disease prevention using evidence-based practice.
• Develop and implement individualized plans to achieve optimal population health and disease management.
• Demonstrate leadership and collaborate with the interdisciplinary team.
• Order and interpret labs, tests, and diagnostic studies.
• Prescribe pharmacologic and non-pharmacologic interventions.
• Initiate referrals and consults to specialty services.
• Provide education to patients and their families.
• Assess and adjust the plan of care as needed to achieve optimal patient outcomes.
• Align care within the GEC/Home Based Primary Care Program, including health promotion, health prevention, geriatric care, primary care, and palliative care.
• Deliver competent care in the home of Veterans with complex needs; utilize interventions to maximize health capabilities and reduce unnecessary urgent care and emergency room visits, per Veteran s goals of care.
• Make home visits, Veteran Video Connection (VVC) visits, and telephone visits to Veterans in the HBPC program.

• Two years of experience as a Nurse Practitioner.
• Prescriptive Authority.
• Active valid state driver s license.

• Work Schedule:
  
  Monday–Friday, 7:30am – 4:00pm.
