Care Talk Health seeks an experienced Registered Nurse to join our virtual care team on a remote, contract (1099) basis. You will support care management, care navigation, and AWV chart preparation across CCM, RPM, RTM, and APCM programs, while closing care gaps and coordinating orders.
This role requires strong clinical judgment, excellent documentation, and the ability to work independently across platforms. Expect 40 hours weekly with EST-based schedule, including some weekend coverage.
