Summary
The Nurse Practitioner (Home Based Primary Care) practices in Geriatrics and Extended Care Service to provide a full range of services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission. The Nurse Practitioner (NP) provides advanced clinical practice, consultation, and management.
Duties- Major duties include but may not be limited to:
Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence‑based practice - Develops and implements an individualized plan to achieve optimal population health and disease management
- Demonstrates leadership and works collaboratively with the interdisciplinary team
- Orders and interprets labs, tests, and diagnostic studies
- Prescribes pharmacologic and non‑pharmacologic interventions
- Initiates referrals and consults to specialty services
- Provides education to patients and their families
- Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes

- Provides care within the GEC/Home Based Primary Care Program including health promotion, health prevention, geriatric care, primary care, and palliative care
- Delivers competent care in the home of Veterans with complex needs, utilizing interventions to maximize health capabilities and reduce unnecessary urgent care and emergency room visits per Veteran's goals of care
- Makes home visits, Veteran Video Connection (VVC) visits and telephone visits to Veterans in the HBPC program
- Preferred Experience:
Two years experience as a Nurse Practitioner – Prescriptive Authority – and an Active Valid state driver's license - VA offers a comprehensive total rewards package: VA Nurse Total Rewards Pay:
