Overview

Coverage area:
Somerset County - Somerville (08876), Bridgewater and Hillsborough;
Phillipsburg (Warren County); plus nearby sites in Edison, Clark, Westfield, Plainfield, Elizabeth and Rahway.

Provide high-touch palliative care for people facing serious illness across SNF, home, and community settings — with a structured ramp into palliative care, physician/APP backup, and a team that does not leave you to figure it out alone.

Base salary $130,000–$160,000/year plus annual performance bonus up to 12.5%. Primarily a Monday–Friday schedule, 28 paid time off days in year one, a $0/month employee-only medical option, malpractice coverage, license/DEA support, CME, and tuition support.

NJ Palliative Care Solutions is on a mission to deliver palliative care without walls across New Jersey, partnering with skilled nursing facilities, home-based providers, and leading health systems. We are hiring an experienced Nurse Practitioner or Physician Assistant to provide outpatient and SNF-based palliative care across Central New Jersey for patients living with serious illness. This is our experienced-provider track: 2+ years of palliative, hospice, or other serious-illness care is required.

What you’ll do:

  • Perform at the top of your license, performing best practice, holistic, patient-centered, evidence-based interdisciplinary palliative medicine.

  • Provide palliative care consultations and follow-up visits across skilled nursing settings, including short-stay subacute/rehab patients with recent hospitalizations, high symptom burden, or complex goals-of-care needs, as well as ongoing management of long-term care residents.

  • Deliver palliative assessments and follow-up visits in SNFs and community settings.

  • Manage complex symptoms for patients with advanced illness and chronic disease.

  • Lead goals-of-care and advance care planning conversations with patients and families, coordinating with partner home health and/or hospice agencies if and when appropriate.

  • Partner with the interdisciplinary team and facility staff, primary care, and hospice.

  • Support transition planning across the SNF stay – from admission after hospitalization through discharge home, return to long-term care, rehospitalization decisions, hospice transition, or other next-site-of-care planning.

  • Complete timely documentation aligned with clinical, billing, and quality standards.

  • Travel locally within Central NJ, with mileage reimbursement and clinical/EMR support provided.

What you bring:

  • Active New Jersey Nurse Practitioner or Physician Assistant license and national board certification.

  • DEA + CDS registration, or ability to obtain by start date.

  • 2+ years of palliative care or closely related serious-illness experience required (hospice, palliative, SNF/LTC, oncology, geriatrics).

  • Palliative care certification (e.g., ACHPN / CHPN) preferred.

  • Strong communication, clinical judgment, compassion, and comfort working on interdisciplinary teams.

Compensation & total rewards:

  • Competitive base salary: $130,000–$160,000/year.

  • Annual performance bonus up to 12.5% of base salary, rewarding both productivity (RVUs) and quality of care.
