Bluegrass Care Navigators is seeking an After-Hours Hospice RN to deliver compassionate, patient-centered care outside of regular business hours. The role focuses on home-based visits, symptom management, and clinical guidance for patients and families in their own homes and in care centers as needed.
The RN will work independently, coordinating with an interdisciplinary team to ensure continuous, high-quality hospice care during evenings and weekends.
