Remote RN Case Manager (Nationwide | Monday–Friday, No Weekends)

• 100% remote – work from anywhere in the U.S.
• Day shift schedule (Mon–Fri) 8:00 a.m. to 4:30 p.m. in your time zone.
• Occasional late shift rotation until 8:00 p.m.
• No weekends or holidays.

• Assess, plan, and coordinate care to support members’ overall wellness.
• Use clinical judgment and data to identify risks and remove care barriers.
• Develop personalized care plans addressing complex and co‑morbid conditions.
• Collaborate with multidisciplinary teams to optimize outcomes.
• Engage members through strong telephonic communication and coaching.

• Active, unrestricted RN license (Compact license preferred).
• Willingness to obtain additional state licenses (company paid).
• 3+ years acute care RN experience (med/surg, ICU, or post‑surgical).
• Strong assessment, communication, and critical thinking skills.
• Travel of less than 5% may be required for clinical audits.
• Education:
  
  Associate’s Degree in Nursing required (BSN preferred).

• Preferred case management or home health experience (1+ year).
• Compact RN licensure.
• CCM certification.
• Proficiency with Microsoft Office tools.
• Strong telephonic communication skills.

• Work hours:
  
  40 hours per week, full‑time.
• Pay range: $60,522.00 – $ (base salary depends on experience, education, geography, etc.).
