Alignment Health is seeking a fully remote RN Case Manager to support SNP members by phone, working Pacific Time hours, Monday–Friday, 8:00 AM – 5:00 PM PT. California RN licensure is required.
In this role you will conduct comprehensive health assessments, create individualized care plans, coordinate care with physicians and specialists, educate members and caregivers on disease management and preventive care, monitor progress, and advocate for timely interventions.
