Join our life‑saving transplant team as a Transplant Coordinator RN, where you'll play a vital role in guiding patients through their pre‑ and post‑operative transplant journey while serving on our multidisciplinary selection committee to assess transplant candidacy. We’re seeking an experienced RN (transplant experience and CCTC certification are highly valued) to manage comprehensive patient evaluations, facilitate waitlist coordination through the national registry, and collaborate with our robust team of surgical providers, nephrologists, pharmacists, dietitians, and social workers using Cerner Power chart, OTTR, and Microsoft Teams.

This position provides comprehensive management and coordination of care for patients requiring kidney, pancreas, liver, heart, lung or stem cell transplantation. The level and intensity of care coordination provided is situational and appropriate based on patient need and payor requirements.
