Carease Health - MI is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

As a Nurse Practitioner at Carease Health - MI, you will have the opportunity to make a real difference in patient care while working in a dynamic and supportive clinical environment. This is a part-time position of 18 hours per week.

• Providing high-quality preventative care to residents in skilled nursing facilities through on-site visits and remote work
• Building strong relationships with the facility staff and residents
• Maintaining a patient-centered approach and being a collaborative team player
• Documenting patient care using a dedicated scribe team

• Active NP or PA license (in the state of practice)
• 1+ year of experience in Geriatric care and documentation review
• Excellent communication, collaboration, and teamwork skills
