Field Advanced Practice Provider - NP - Metro Detroit Suburbs

Work Location

Field-based role enabling associates to primarily operate in the field, traveling to client sites or designated locations as required, with occasional office attendance for meetings or training. The approach ensures flexibility, responsiveness to client needs, and direct, hands‑on engagement.

The successful hire will be a Michigan-licensed Nurse Practitioner and will provide in‑home and virtual remote palliative care in the Metro Detroit suburbs area. Working hours are 8 am–5 pm and visits are clustered by area. Qualified candidates in the Lansing area are encouraged to apply.

• Provide services in home settings to care for patients.
• Develop and implement clinical plans of care for adult patients facing chronic and complex conditions, including co‑morbidity and limited personal resources.
• Gather history and perform physical examination and diagnostics as needed, then develop and implement treatment plans based on the patient’s goals of care and current conditions.
• Meet the patient’s and family’s physical and psychosocial needs with support and input from the company’s inter-disciplinary team.
• Educate patients and families about medication usage, side effects, illness progression, diet and nutrition, medical adherence, and crisis anticipation and prevention.
• Maintain contact with other clinical team members, patients’ other physicians and providers to coordinate optimal care and resources in a timely manner and in accordance with state regulations and company standards.
• Maintain patient medical records and documentation consistent with state regulations and company policies.
• Participate in continuing education as required by state.
• Prescribe medication as permitted by state prescribing authority.

• MS in Nursing.
• Current unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in applicable state(s).
• Possession of DEA registration or eligibility.
• CPR certification.

• At least 2 years of experience in home-based services, complex care cases, hospice and/or palliative care; bedside hospital, oncology, emergency room (ER), or intensive care unit (ICU) experience highly preferred.
• Experience with Electronic Medical Records (EMR).
• Bilingual in English/Spanish.
• Satisfactory completion of a Tuberculosis test (required for Carelon Health business unit).
• Travel may be required.
