We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our team at Carease Health - NY. As a leading provider of remote patient monitoring (RPM), chronic care management (CCM), and collaborative care management (CoCM), we prioritize proactive measures for enhanced physical health and well‑being.
About the RoleAs a Nurse Practitioner, you will play a crucial role in delivering high‑quality preventative care to residents in skilled nursing facilities. This hybrid role combines on‑site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. You will build strong relationships with the facility, maintain a patient‑centered approach, and be a collaborative team player.
Responsibilities- Provide high‑quality preventative care to residents in skilled nursing facilities through on‑site visits and remote monitoring
- Partner with a dedicated scribe team for efficient documentation
- Build strong relationships with the facility and maintain a patient‑centered approach
- Collaborate with a multidisciplinary team to develop and implement individualized care plans
- Conduct regular assessments and evaluations to ensure optimal patient outcomes
- Active NP or PA license (in the state of practice)
- 1+ year of experience in Geriatric care and documentation review
- Excellent communication, collaboration, and teamwork skills
- A hybrid work schedule that allows you to make your own schedule
- This is a part‑time position of $33,000 - $35,000 annually
