Humana Inc. is seeking a Telephonic Registered Nurse Care Manager to support patients transitioning into and out of our services. You will work remotely with the Center Well Home Health clinical team and help coordinate post-acute care options, including home health, hospice, and palliative care.
You will leverage clinical data, communicate with empathy over the phone, and assist in admissions while adhering to HIPAA and company policies. Remote work with periodic training may be required.
