Position Overview
Colorado State Veterans Community Living Center at Rifle located in the Colorado River valley. This is a part‑time nurse position. The employee is required to work at the assigned work location during scheduled work times: 6:00 pm – 6:00 am on Thursdays, Fridays, and Saturdays (work arrangements are subject to change at any time).
Responsibilities- Perform admission and daily assessments during clinical stay, identifying critical stays, incidents, emergencies, acute illnesses, and behavioral changes with RN oversight.
- Identify and implement appropriate nursing interventions, treatment plans, and physician referrals.
- Document current, accurate, and pertinent information regarding care and treatment provided to individual residents.
- Indicate specific changes in status, medications, and physician orders.
- Detect abnormalities in a resident’s health status as indicated by symptoms, vital signs, and laboratory results, and report information to the physician.
- Drive a State vehicle to conduct off‑site pre‑admission assessments or take a resident to a doctor or hospital if not transported by ambulance.
- Require an RN co‑sign on admission assessments.
- Direct and implement nursing care to residents, ensuring compliance with accepted standards, departmental and facility policies, VA and State of Colorado regulatory requirements.
- Direct subordinates to assure these standards are met.
- Review and audit physician orders, MARS, CNA flow sheets, bathing schedules, and BM sheets to determine compliance with policies and standards of care as part of a department quality assurance activity.
- Competently administer medications within scope of practice while observing the five rights of safe medication administration, resident rights, infection control standards, and appropriate nursing policies.
- Document all medications given and justify scheduled medications not given.
- Appropriately identify changes in resident’s conditions that need to be assessed by a registered nurse.
Perform other duties as assigned.
Minimum Qualifications- Current, valid licensure as an LPN from the Colorado Board of Nursing or any state participating in the Enhanced Nurse Licensure Compact (eNLC).
- Ability to follow patient confidentiality standards.
- Excellent interpersonal and communication skills.
- Ability to read and comprehend the English language.
- Ability to effectively communicate fluently in English, both verbally and in writing.
- Ability to follow care plans.
- Full background check, including CBI & FBI fingerprint check, Colorado Court Database check, Medicare Fraud Database, TRAILS Database check, and a CAPS check if required.
- Must have State of Colorado residency at the time of application and throughout employment.
- Must possess a valid, non‑restricted Colorado Driver’s License or a non‑restricted U.S. Driver's License for positions within 30 miles of the Colorado state border. Out‑of‑state drivers must obtain a Colorado license within 30 days of employment start.
