Ready to make a difference? Join us. We are hiring a Care Manager to be an essential element of an Integrated Care Model, relaying member needs and advocating for the best possible care.
Work schedule:
4 10‑hour days (7 a.m.–6 p.m.). No on‑call or weekend requirements. Remote work is available for applicants located in Mecklenburg County, NC or surrounding areas. Approximately 50‑75% of the time will be spent in the field visiting members at home or in long‑term care facilities.
- Assess, plan and implement individualized care‑management interventions that aim for the most appropriate, least restrictive level of care.
- Identify and initiate referrals for health‑care and community‑based services, including financial, psychosocial, community and state supportive services.
- Develop and implement care‑plan interventions throughout the continuum of care as a single point of contact.
- Communicate necessary health‑related information to all stakeholders to ensure coordinated care and timely service delivery.
- Advocate for members and families to ensure their needs and choices are fully represented and supported by the health‑care team.

Required Qualifications- Current, unrestricted independent licensure as a Registered Nurse.
- Minimum two years of clinical experience.
- At least one year of experience with MS Office (Word, Excel, Outlook).
- Reliable transportation and willingness to travel up to 75% of the time within the assigned territory.
- Residency in Mecklenburg County, NC, or surrounding areas.
- Access to a designated quiet workspace at home that can securely protect Protected Health Information (PHI).
- BSN, master’s degree, or higher in a clinical field.
- CCM certification.
- At least one year of community case‑management experience coordinating care for individuals with complex needs.
- Experience working in team‑based care.
- Background in Managed Care.
- Ability to transition between office and field locations multiple times per day.
- Ability to navigate multiple locations and terrains to visit members and providers.
- Ability to transport equipment (e.g., laptop) to and from visit sites.
- Capacity to remain stationary for extended periods while completing computer or tablet work.

Hourly pay ranges from $29 to $52, based on local labor markets, education, work experience, certifications, and other factors.
