The position travels locally throughout the greater Lebanon, Ohio area (Lebanon, Middletown, Springboro, Dayton and part of Cincinnati) providing patient services in a home‑health care setting.

Full‑time 1st shift (Monday‑Friday 8:00‑5:00) with on‑call responsibility.
Responsibilities- Complete an initial assessment of patient and family to determine home‑care needs; provide a complete physical assessment and history.
- Initiate the plan of care and make necessary revisions as patient status and needs change.
- Use health assessment data to determine nursing diagnosis.
- Establish goals based on nursing diagnosis and incorporate therapeutic, preventive, and rehabilitative nursing actions; involve the patient and family in the planning process.
- Initiate appropriate preventive and rehabilitative nursing procedures.
- Administer medications and treatments as prescribed by the physician.
- Counsel the patient and family in meeting nursing and related needs.
- Provide health‑care instructions to the patient as appropriate per assessment and plan of care.
- Identify discharge‑planning needs as part of the care‑plan development and implement prior to discharge of the patient.
- Prepare clinical notes and update the primary physician when necessary and at least every sixty days.
- Communicate with the physician regarding the patient’s needs and report any changes in the patient’s condition; obtain/receive physician orders as required.
- Communicate with community‑health related persons to coordinate the care plan.
- Participate in on‑call duties as defined by the on‑call policy.
- Ensure that arrangements for equipment and other necessary items and services are available.
- Instruct, supervise and evaluate home‑health aide care provided.
- Review LPN visit records and physician orders and co‑sign.
- Education:
Bachelor’s degree in Nursing preferred. Graduate of an approved nursing program. - Licensure/Certification:
Valid Ohio Registered Nurse (RN) license, possession of and maintenance of current CPR certification. - Experience:
Minimum of 1 year clinical nursing experience; experience in home health or community health/hospice preferred. - Since this is a community‑based ministry, all case managers are required to be licensed drivers with a vehicle in good working condition, insurance in accordance with Ohio laws, and a clean driving record for insurability purposes.
