Rifle, Colorado. The facility provides 89 beds of skilled nursing care, including an 18‑bed Special Care Unit for residents with Alzheimer’s disease or related conditions.

Client Care Aides provide hands‑on care to veterans, their spouses, and eligible “Gold Star” parents, assisting with bathing, dressing, grooming, hygiene, skin care, range‑of‑motion, positioning, ambulating, transferring, toileting, feeding, transportation, basic vital‑sign assessments, charting, and implementation of infection‑control practices. The role supports resident dignity and compassionate care within a safe, infection‑controlled environment.

• Perform basic assessments: vital signs, blood pressure, weight, intake and output, and basic skin evaluation.
• Administer infection‑control procedures, including proper waste disposal.
• Clean and maintain the linen room and equipment; restock supplies and complete supply forms.
• Use safety and positioning devices correctly, including side rails and transfer belts.
• Participate in ongoing staff education programs.
• Execute other duties as directed.

• High school diploma or GED; or current valid CNA license, or ability to obtain one within 120 days of hire.
• Required CPR certification and vaccinations as required by state and facility.
• Knowledge of resident care practices, judgment in emergency situations, and ability to communicate effectively with staff, residents, families, and other health‑care providers.
• Ability to work with an elderly population having varied physical, mental, and emotional needs.
• Demonstrated professional conduct, confidentiality, and strong communication skills.
• Long‑term care or nursing‑home experience is highly desirable.
• Effective problem‑solving skills.

• Experience with specialized client care applicable to the setting.
• Knowledge of quality standards, procedures, and protocols.
• Ability to initiate, perform, and complete assigned duties in resident care.

• Full background check: name and fingerprint, court database, Medicare fraud database, reference checks, license verification, drug screen, and CAPS.
• Mandatory TB test.
• State of Colorado residency at application time and throughout employment (unless stated otherwise).
• Valid, non‑restricted Colorado driver’s license required for certain positions; out‑of‑state drivers must obtain a Colorado license within 30 days of employment.
