Alignment Health is seeking a remote bilingual Spanish RN Case Manager for Transitions of Care to join the outpatient case management team. California RN license required; minimum five years of clinical case management experience preferred.
This telephonic role coordinates care plans, authorizations, and referrals, interfaces with clinicians, monitors outcomes, and educates patients and families on options. Work schedule is Monday to Friday, 8:00 AM–5:00 PM Pacific Time.
