Health Snap is seeking a licensed practical nurse (LPN/LVN) to support patients in chronic care management and remote patient monitoring across multiple states. This full-time, 40-hour-per-week role is Monday–Friday with Alaska license required and pay aligned to the resident state.
You will perform phone consultations, educate patients, document care plans, and respond to remotely transmitted data per approved workflows, using EHR and care management software.
