Registered Nurse Case Manager

Job Description

We are a Maryland DDA provider agency seeking a residential Registered Nurse Case Manager/Delegating Nurse to join our team! This is a field position primarily working from a home office with some minimal time spent at our corporate offices. You will be responsible for overseeing the health care needs of an assigned caseload of persons with intellectual/developmental disabilities.

• Complete initial nursing assessment of new customers
• Conduct on-site supervisory visits and assessments of customers on your caseload minimally every 45 days
• Provide training and ongoing supervision of unlicensed personnel in the administration of medication, treatments and nursing care
• Establish, update and oversee implementation of the nursing plan of care
• Provide case management, serving as a liaison between the customer and their health care providers
• Provide on-call telephone triage support on a rotational basis
• Attend interdisciplinary meetings as needed
• Attend required Maryland DDA trainings
• No weekends or holidays involved, with the exception of the rotational on call telephone triage

• Experience in developmental disability nursing/delegating nursing preferred
• Strong critical thinking and assessment skills
• Comfortable and adept at using technology and eMAR systems
• Excellent communication and interpersonal skills
• Training/supervision experience preferred
• Ability to thrive in a fast-paced environment
