Flagler Health is seeking a dedicated LPN or LVN to serve as the on-site contact for Medicare 855B site inspections and to support the Chronic Care Management program at our Wisconsin facility.
The role is currently five days a week on-site at 444 Reid Street, De Pere, WI 54115, with a planned hybrid schedule as the program grows. On-site presence from 10:00 AM to 2:00 PM is required and HIPAA compliance is essential.
