Job Overview

Nurse Chronic Care Coordinator (Remote). The role involves telephonic encounters with patients on behalf of our partners each month, developing detailed care plans within the electronic health record. This initially begins as an independent 1099 contractor position with potential to transition to full‑time W2 employment, including employee benefits.

• Conduct telephonic encounters with patients and develop detailed care plans using our EHR templates.
• Maintain HIPAA‑compliant practices while working from a private home office.
• Set your own hours within a minimum commitment of 20 hours per week (typically 20‑30 hours).
• Manage a patient panel of at least 100 patients per calendar month and complete encounters for at least 90% of assigned patients.
• Adhere to the plan of care and orders of the practice, providing preventive and motivational coaching for multiple chronic conditions.
• Attend scheduled meetings (bi‑monthly staff meetings, monthly one‑on‑ones, etc.).
• Participate in productivity‑based pay structure: $10.00 per encounter up to 99/month, $10.25 from 100‑149, $12 from 150‑199, $14 from 200‑249, $16 for more than 250 encounters; payment tiers require three months of consistency.
• Spend a minimum of 20 minutes per encounter (time cumulative).

• Graduate from an accredited School of Nursing (LPN, LVN, RN, BSN, etc.).
• Current COMPACT license to practice as an RN, LVN, or LPN in your state of residence with no disciplinary actions.
• Minimum of two (2) years of clinical experience in Med/Surg, Case Management, and/or home health care.
• Hands‑on experience with Electronic Medical Records and proficiency in Windows desktop applications (Microsoft Office 365, Teams, Excel).
• Ability to exercise initiative, judgment, organization, time‑management, problem‑solving, and decision‑making skills.
• Strong verbal, written and listening skills with clear diction and exemplary phone etiquette.
• High‑speed Internet and a desktop or laptop computer (Windows or Mac OS required; Chromebooks not allowed).
