Job Summary:We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.
Responsibilities:- Build strong relationships with facilities and maintain a patient-centered approach
- Collaborate with a dedicated team to provide proactive and comprehensive care
- Demonstrate excellent communication, collaboration, and teamwork skills
- Active NP or PA license (in the state of practice)
- 1+ year of experience in Geriatric care and documentation review

-Time position of 7 hours per week.
Salary: $15,000 - $17,000 ANNUALLY (to be discussed during the interview process)
