Aspirus Wisconsin Rapids Hospital & Clinics is seeking a RN
- Ambulatory Care Coordinator to join our Care Coordination team!
The Ambulatory Care Coordinator is a Registered Nurse (RN) directly responsible for assessing, coordinating and integrating healthcare for patients with chronic conditions in the Aspirus Clinic System. This RN position manages all aspects of patient centric care coordination through collaboration with physicians, staff and other health professionals to provide services across the health care continuum for all patients within the clinic setting.
The Ambulatory Care Coordinator will work directly with the entire health care team to deliver innovative population health management practices to the patients and community of Aspirus.
HOURS: Full-Time or 1.0 FTE, 80 hours every pay period, Day only
This is a Hybrid position with the possibility of 3 days work from home and 2 days in the office.
Experience/Qualifications- Knowledge of patient care standards normally acquired through graduation of an accredited Registered Nurse program.
- Five years' experience in a clinical environment and possesses extensive experience in chronic care and population health management.
- Possesses an active Michigan or Wisconsin RN nursing license.
- Current BLS certification or obtained within 90 days of hire
- All clinical personnel in identified job codes are required to maintain Basic Life Support (BLS) training through independent study, manikin and knowledge testing. An acceptable BLS training program is the Basic Life Support (BLS) Health Care Provider course from the American Heart Association or the American Red Cross Professional Rescuer Program with AED. (See Cardiopulmonary Resuscitation Training Policy). - Skills in organizing and prioritizing work along with multi-tasking simultaneously.

- Ability to demonstrate a strong, autonomous work ethic with qualities of personal and professional integrity.
- Detail and goal oriented demonstrated by personal initiative and accountability.
- Possesses confidence to communicate and outreach to other community health care organizations and personnel.
- Possesses proficient computer skills, including Microsoft Office (specifically Word and Excel).
