Cadence is a clinical AI company delivering virtual chronic care support for Medicare patients enrolled in Cadence programs. The Clinical Navigator (LPN/LVN) will provide remote coaching, education, and care coordination to manage chronic conditions and improve outcomes.
This full-time remote role follows Pacific hours with standard 8/9–5/6 PM schedules. You will document care in Cadence platform and collaborate with clinicians to drive effective care delivery.
