Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN) Home Health Pittsfield, MA

Valid MA LVN/LPN license, CPR required - Experience:
Prior home health or community nursing experience preferred - Pay Rate: $50.25/hour
- Benefits:

- Provide and document nursing care to clients in their homes
- Collaborate with healthcare team members and community resources to coordinate patient care
- Evaluate patient responses to interventions and adjust care plans accordingly
- Follow established protocols and policies autonomously under general supervision
- Assist in promoting health, preventing illness, and supporting rehabilitation efforts
- Maintain accurate clinical records and communicate effectively with team members
- Be available for on-call responsibilities and flexible with work hours as needed
- Valid Massachusetts LVN/LPN license
- CPR certification
- 1+ years of relevant nursing experience, preferably in home health or community care
- Strong critical thinking, interpersonal, and communication skills
- Ability to exercise independent judgment and maintain confidentiality
- Reliable transportation and ability to drive in various weather conditions
- Willingness to accept on-call duties and adhere to performance standards
- Flexibility regarding work hours, with no more than 5 days of requested time off during the assignment
