What You ll Do:
  • Provide and document nursing care to clients in their homes
  • Collaborate with healthcare team members and community resources to coordinate patient care
  • Evaluate patient responses to interventions and adjust care plans accordingly
  • Follow established protocols and policies autonomously under general supervision
  • Assist in promoting health, preventing illness, and supporting rehabilitation efforts
  • Maintain accurate clinical records and communicate effectively with team members
  • Be available for on-call responsibilities and flexible with work hours as needed
What You Need:
  • Valid Massachusetts LVN/LPN license
  • CPR certification
  • 1+ years of relevant nursing experience, preferably in home health or community care
  • Strong critical thinking, interpersonal, and communication skills
  • Ability to exercise independent judgment and maintain confidentiality
  • Reliable transportation and ability to drive in various weather conditions
  • Willingness to accept on-call duties and adhere to performance standards
  • Flexibility regarding work hours, with no more than 5 days of requested time off during the assignment
