Schedule: Full time, Weekdays on day shift

• Graduate of an accredited LPN/LVN school of nursing.
• Within 30 days of hire:
  National Provider BLS
  - American Heart Association.
• Upon hire:
  Oregon Licensed Practical Nurse.
• Driving is a necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information.
• 1 year Experience in nursing; long term care, acute care, ambulatory care or home health and hospice care.

• Experience in home health, hospice or public health.

Pay Range: $31.35 - $43.33 per hour. Additional compensation may be available for this role, such as shift differentials, standby/on‑call, overtime, premiums, extra shift incentives, or bonus opportunities.

Work Location: Providence Health Plaza (HR) Bldg 1 - Portland, Oregon. (Remote work arrangement available.)
