Location: Ladysmith
Humana is seeking an RN Care Manager to coordinate care for Wisconsin Family Care members, with no night or weekend shifts and a remote-home work setup covering Rusk County, WI. You will assess needs, develop MCPs, educate members, and collaborate with providers, traveling to in-person visits as required.
Qualified candidates hold an Associate degree in nursing and Wisconsin RN licensure, with prefered 1 year of care management experience and residence within 45 minutes of the coverage area.
