RN Home Health Visits Shively​/Valley Station

Coverage Areas

KCW 40214, 40215, 40216, 40258, 40272, 40118 and possibly areas within Metro Louisville

One weekend per month to perform any SOCs and on-call visits.

HCHB/Oasis. Home Health experience and Home Care Home Base/HCHB required;
Oasis preferred but not required.

The Home Health Registered Nurse is responsible for providing and documenting skilled one‑on‑one nursing care in accordance with the physician‑ordered individualized patient care plan. This care is provided mostly in the patient’s home or a residential setting but may be transferred outside the home during appointments and other activities the nurse may accompany the patient to.

• Graduate of an accredited school of nursing
• Currently licensed as a Registered Nurse in the state in which the RN will practice
• Current CPR if applicable
• TB questionnaire, PPD or chest x‑ray if applicable
• Current Health certificate (per contract or state regulation)
• Must meet all federal, state and local requirements
• Must be at least 18 years of age
