Sentara Health Plans is seeking an Integrated Case Manager, RN. This remote role requires residency in Suffolk, Norfolk, VA Beach, Portsmouth and surrounding areas with travel up to 1 hour to see members approximately 50–75% of the week.
The Integrated Case Manager develops and monitors care plans, performs clinical assessments, coordinates physical and behavioral health services, and collaborates with the health care team to optimize member health across the continuum.
