Complex Care Solutions (CCS) is seeking a Nurse Practitioner per diem to deliver comprehensive home and virtual assessments for health plans nationwide. You will spend up to an hour with each member to understand personal needs and determine care requirements to keep them at home and out of the hospital.
The role requires NP certification, unencumbered license, CPR, and HIPAA compliance, with experience or comfort in CMS/NCQA guidelines and EHR tools.
