Complex Care Solutions (CCS) seeks a Nurse Practitioner per diem to perform in-home and telehealth assessments for members in Clinton County, IA. You will spend up to an hour with each member to capture a comprehensive view of needs to keep them in the home and out of the hospital.
Responsibilities include reviews, documentation, and collaboration with PCPs and health plans, with respect to CMS/HEDIS guidelines, and adherence to HIPAA and company policies.
