CVS Health is seeking a remote Case Manager RN to join the Costco Team, delivering care coordination from home with a standard 40-hour workweek (8 AM–5 PM CST, occasional 10 AM–7 PM). You will assess needs, plan care, coordinate resources and advocate for patients to improve health outcomes.
The role requires an active multi-state RN license, 3+ years of clinical RN experience, and proficiency with MS Office and EHRs. Willingness to obtain additional state licenses is supported by the employer.
